Guided by a person-centered wellness model, we focus on supporting the whole individual—while delivering the highest quality of care for our residents. We’re seeking an MDS Coordinator who shares our commitment to wellness and has a passion for ensuring accurate and comprehensive resident assessments. In this part-time role, you’ll play a pivotal role in our care team, coordinating the assessment process, working closely with an interdisciplinary team, and ensuring compliance with all regulatory requirements. Your expertise will help us maintain the highest standards of care and optimize outcomes for our residents in a collaborative, positive environment.

ESSENTIAL FUNCTIONS

  • Conduct and coordinate the development and completion of the resident assessment (including the delegation of specific tasks) (MDS) in accordance with current rules, regulations, and guidelines that govern the resident assessment, including the implementation of CAAs. Evaluate each resident’s condition and pertinent medical data to determine any need for special assessment activities or a need to amend the admission assessment.
  • Coordinate the development of a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident, indicates the care to be given, goals to be accomplished, and which professional services is responsible for each element of care. Creates diagnosis list using appropriate ICD-10 codes.
  • Follows policies and practices ensuring consistency with current standards promulgated by appropriate local, state, and federal regulatory bodies.
  • Participates in a quality assessment/performance improvement program.
  • Identifies and reports deviations from safe practice/risk management issues.
  • Is competent in acting in emergency situations based on Safety Manual, Internal and External Disaster Preparedness, and other policies.
  • Complies with Mather Code of Conduct, Policies, Compliance Program, and all applicable laws and regulations in performance of job duties; reports all known and suspected violations to supervisor/manager, Mather Compliance Officer, HR representative, or the confidential employee helpline.
  • Establishes, operationalizes, engages team members in, and sustains a Quality Assurance Process Improvement process.
  • Applies the concepts of the Mather Wellness Model and the 3 A’s in the care of residents: Achievement, Autonomy, and Affiliation.
  • Documents care per documentation policy and standards.
  • Assists in licensing and survey preparedness and processes.
  • Responsible for covering shifts as a staff nurse when required.
  • Uses a customer-focused approach to problem-solving philosophy.
  • Maintains a courteous and professional manner in interactions with others.
  • Communicates with residents and families in a timely manner.
  • Uses discretion in handling confidential information, following all federal, state, and local privacy and confidentiality requirements.
  • Interactions with employees, residents, resident representatives, and guests will reflect commitment to inclusivity.
  • Demonstrates understanding of and commitment to the company Mission, Core Values, and Nextraordinary philosophy.
  • Create exceptional experiences for internal and external customers in every interaction.
  • Operates efficiently within established parameters to maximize available resources.
  • Participates in sales and promotion of the community.
  • Incorporates fiscal responsibility through efficient, safe, cost-effective practices in the delivery of care.
  • Increases personal growth and development through formal and informal education programs.
  • Facilitates teamwork between functional departments to promote effective operations.
  • Participates in employee wellness opportunities.
  • Responsible for keeping abreast of changes in evidence-based practice, regulatory requirements, and professional regulations.

QUALIFICATIONS AND SKILLS

Required:

  • Effective staff development and coaching abilities.
  • Excellent interactive and communication skills.
  • CPR/AED certification.

Preferred:

  • RN with RAC certification and/or proven MDS/PPS reimbursement experience.
  • Knowledge related to ICD-10 coding.
  • Knowledge of related federal and state regulations.
  • Computer proficiency.
  • Two years MDS and nursing/leadership experience.
